Crash Report?

Is there a crash report to be filled by the pilot (luckily we have this advantage)?
I wonder if having the report filled out could help us to pinpoint specific areas where we could train our pilots.

Good pilots keep their number of landings equal to their number of takeoffs. Takeoffs are optional but landings are Mandatory.

The crashreport i think is to give the hub-manager an idea of what happened and to assist him in giving the pilots some help to avoid further crashes.

The crash report is there to help the HC to decide how long to put the pilot on suspension for.
If we feel the need, we can write advice in the email we send
