Yes - 4500FPM at CLB-2 has happened to me lots of times. Usually on IVAO when the wind changes suddenly and VNAV pitches up to maintain the speed.

My aircraft was loaded with about 1/2 total carco capacity and 3/4 PAX (in the PMDG Aircraft loads program).

Ohh, didn't think about that. Not surprised then, those brutal wind changes are a real pain. Causes overspeed too...
There is a wind transition smoothing option, if not already, try it out.

Its on already :[ Haven't had an overspeed yet... the PMDG autopilot is very fast to react and pulls the nose up quicly, making 4500FPM.
