Unfortunantly it has a pretty bad safety record - about 60 aircraft lost in about 30 years plus a lot of incidents - but I think most were caused by the extreme conditions they fly in, poor maintenace and sometimes human error, rather then design flaws.
One very disturbing accident involved a Tu-154B-1 flying for TAROM (Romania's national air transporter), reg YR-TPH: due to bad visibility, the plane touched down 300m short of the runway at Nouadhibou Airport. The crew thought they were on the runway, but they were actually in the water. Everyone survived the crash-landing, but when the passangers tried to reach the shore, some (1-3 different sources say) were killed by sharks. Trully disturbing.
